I generally ignore it. But, in fact, I hate when commenters add weird, unrelated, and insignificant images to their comments.
It's stupid and pointless. I think they just want to be noticed. But it's a pretty lousy strategy.
I am also not a big fan of downvote or flags, but if you want some reward or you want to get noticed, at least do some effort like others are doing.
