A teacher’s job is to teach and make the student comfortable in asking and raising questions that they have in mind. If a student is feeling uneasy in asking questions then, in my view, the teacher has failed to deliver the job, unless, the student has his/her own issues.

This bit is SUPER important. If a teacher fails on this, then he fails basically on everything.
