He cumplido 3 de las 73 guardias que tendré a lo largo de todo este año, y sólo puedo decirles ¡WOW! me ha llegado de todo, han habido guardias más movidas que otras, pero ninguna como la última.

Les cuento, me llegó un caso bastante emotivo, que me hizo pensar ¿Cómo alguien puede tener tanta maldad?. Pues verán, el 30/12/21 alrededor de las 4 de la tarde, llegan al ambulatorio funcionarios de la Policía Municipal de los Guayos, y con ellos en brazos, una Recién Nacida, de aparentemente horas de nacida, aún unida a la placenta.

Well, you see, on 12/30/21 around 4 in the afternoon, officials of the Municipal Police of Los Guayos arrive at the ambulatory, and with them in their arms, a Newborn, apparently hours old, still attached to the placenta .

Ellos cuentan que a eso de las 3:45 pm, se les notifica por parte de algunos vecinos de la localidad, la permanencia de la recién nacida en una bolsa de basura, ubicada en un terreno baldío.

They say that around 3:45 pm, they are notified by some residents of the town, the permanence of the newborn in a garbage bag, located in a vacant lot.

Al llegar al ambulatorio, y examinarla, observamos que tenía heridas cortantes en región cervical derecha; o sea, no bastó con abandonarla sino que aunado a eso, intentaron asesinarla.

Upon arriving at the outpatient clinic, and examining her, we observed that she had cutting wounds in the right cervical region; In other words, it was not enough to abandon her but in addition to that, they tried to assassinate her.

Afortunadamente, la bebé, está bien, llegó en buenas condiciones generales, y fue trasladada al hospital Central de Valencia. Pero sin duda alguna esto ha sido lo más fuerte que me ha tocado ver, desde mi paso como estudiante, hasta hoy en día como Médico.

Por respeto a la Recién Nacida, por ser menor de edad y por supuesto lo delicado del caso, me abstengo de colocar fotos.

Hubo 3 traslados, incluido el ya antes mencionado, los otros dos fueron una gestante primigesta, que se encontraba en trabajo de parto, la última paciente fue una señora de 80 años de edad, que saturaba a 80%. Afortunadamente, mi compañero y yo salimos victoriosos de esa guardia, bastante cansados, pero felices, de haberla culminado.

Otro caso bastante peculiar que llegó, no en mi guardia, pero sí en la de uno de mis compañeros, fue el de esta paciente de 50 años de edad, quien padece una patología bastante rara llamada Síndrome de Chediak- Higashi; siendo el único caso en Latinoamérica registrado actualmente.

Another rather peculiar case that came, not on my watch, but on that of one of my colleagues, was that of this 50-year-old patient, who suffers from a rather rare pathology called Chediak-Higashi Syndrome; being the only case in Latin America currently registered.

Este síndrome consiste en una mutación de un gen llamado LYST, afectando el sistema inmunitario, ya que no se crean Fagolisosomas en los Neutrófilos (principal leucocito, indispensable en la defensa contra agentes patógenos) impidiendo así, la lisis bacteriana. Estos pacientes son susceptibles a infecciones recurrentes, albinismo parcial, hepatomegalia, etc; tal cual como era el caso de esta paciente, ya que ingresó al ambulatorio para recibir tratamiento antibiótico, ya que poseía un absceso en región anterior de brazo derecho.

This syndrome consists of a mutation of a gene called LYST, affecting the immune system, since Phagolysosomes are not created in Neutrophils (the main leukocyte, essential in the defense against pathogens), thus preventing bacterial lysis. These patients are susceptible to recurrent infections, partial albinism, hepatomegaly, etc; as was the case with this patient, since she was admitted to the outpatient clinic to receive antibiotic treatment, since she had an abscess in the anterior region of her right arm.

Fue bastante curioso este caso, pero no hizo estudiar a todos, y aprender algo nuevo, algo que desconocíamos completamente.

This case was quite curious, but it did not make everyone study and learn something new, something that we were completely unaware of.
