As Nika enters the reduced-security facility, she encounters a diverse group of patients, each battling their inner demons. Malcolm, another patient, suffers from dissociative identity disorder, mirroring the chaos that Chucky embodies. The great irony here is the collective disbelief around Chucky’s threat; Nika insists that she isn't to blame for her family's deaths, but no one believes her, including Dr. Foley, the psychiatrist who seems more interested in his fame than his patients’ well-being.
Angela, a ghostly apparition, and Malcolm provide both companionship and escalating fears in the sanatorium. As Chucky's influence begins to permeate the establishment, unsettling events escalate, from the bizarre behavior of the dolls to the horrific aftermath of each encounter.
